Abstract (English)

Patients leaving intensive care and their loved ones experience a lack of continuity of care. They seek for a point of contact; they lack someone to fall back on. Moreover, many patients then struggle with physical, cognitive and psychological complaints. Through research by researcher Sabine op 't Hoog, in 2019 the ETZ started ICU nurses who support their colleagues24/7 on general wards. This thesis shows that this does not reduce the number of readmissions to the ICU, nor the risk of dying during hospitalization. However, the study does show the added value of low-threshold collaboration between ICU and other nurses. The coaching and sharing of knowledge contributes to the continuity and further improvement of care (processes). Another important outcome of the dissertation is the added value of telephone aftercare by IC nurses. By using this form of e-health, patients and their loved ones are better supported in the transition to the home situation.
